0165(2)(c) in the infant room - two telephone cords were loose and not secure to the wall. 0165(2)(d) in the infant room - a plastic bag was accessible next to the changing table, teacher removed during the facility inspection on 5/4/2022 and will be inaccessible to children. 0165(3)(g) in the toddler and preschool room: multiple shelves have tape residue.
